Need help with Application!?

I'm 46, Married with a 10 year old Daughter. I have been a stay at home mom for the last 10 years and now when my Daughter goes back to school I need to get a job. My question is when filling out a Application do I put my last job down from 11 years ago or do I put that I was a stay at home mom? Please help
