A time when you have overcome a difficult situation or problem?

Well, I cannot answer them for you - you have to search your background and find an answer that suites the questions.  Let's see if I can help based on you:

So, when did you take ownership of a problem?  

Are you a student?  If so, were you involved in a club that had a fund raising issue?  A deadline that was not going to get met?  Anything like that - what did you do to get them out of the pickle?  

If you are working somewhere:  Was there a mistake that was made, you may have played a small role in the mistake but you took the blame w/o issue.  What was the issue, what went wrong, and what would you have done differently?

Difficult situation:  Were you challanged by a customer?  A co-worker?  The database system at work?  What was the issue and what did you do to overcome the challange.
