you need to go to the nearest health department and state you need a new one and they will send off for it. It will take a week or to to return. GOOD LUCk!

Well you first need to contact you local embassy and get the proper documents

you can go to your local registry office to apply for it? they will give you the forms then we send it off? there maybe a fee for it but thats how i had to do mine? but appy for the full certificate as they do not accept the the shorts any more? so get the full one as you will not be able to get your passport etc?
