Newspaper Adver format for change of name in India?

Can anyone help me with the actual text to be published in the newspaper for name change in India?

u need to first change it with a notory public, who is considered as representative of the high court of the state,

then u need to advertise it for the sake of public at large so that people dealing with u do not in future sue u for any reasons for havng not known or for mistaken identity.

Dear Gary,

The Advertisement is common. It should run as follows:-

" I,.....( your old name ) aged about .. years and residing at.......( your address ) have changed my as ....( your new name ) for all purpose, under the oath taken by Notary.......( name of the Notary )."

This advertisement has to be published in two languages. One in a major circulated English daily and another in any Vernacular daily of your choice. You can approach the news paper office online and submit your application. The charges can be paid by your Credit / Debit card. The news paper Editor may ask you to provide the Affidavit copy for his verification. Send the same by Fax to him. And instruct them when exactly your Add should appear.

Once your Add is appeared in the papers, please collect 10 copies each from your news paper vendor, which you will require, every time you apply for the change in name in all documents.

Go to the news paper office/advertisement agent where you want to put the ad. He will show you a number of such advertisements.
