Ok, so I received my free iPod Touch, now how do I get the rebate?

I ordered a Macbook and I got it on August 5, 2008, along with my free $300 iPod Touch. How do I receive that rebate? Will Apple send me something in the mail, or do I need to call Apple? Please help?
