NO!!!!  you don't have to bring cash for the casino unless you want to.  However, you do not just put your card in the slots or hand it to the dealers at tables.  Here's what you do.  Go to the casino cage where change is made and present your cruise card and request cash.  Some ships have a $2,000 limit on the cash you can get;some even advertise that you can get cash in the casino.  The casino will have you sign for the cash just like any other purchase on your cruise card; thus there are no ATM or transaction fees.  Then you can use the cash to gamble or spend it however you want off the ship.  I usually get cash from the casino rather than using the ship's ATM which charges a fee and then your bank charges a fee also.  

I have done this several times on cruises when I ran low on cash.

You can take cash or use the onboard ATM.   Most on board casinos will not take your on board pass.  I know for sure you have  to have cash for Royal Carribean.  Just keep in mind that the on board ATM's can cost you up to $6.00 in fees.

You should take cash if you plan on to gamble onboard.  You can use the ATM or get a cash advance on a credit card, but they are going to charge you to do that.

You can use your Sea Pass or Sail & Sign card for gambling. Be sure to keep track of your expenditures. You can check your balance anytime at the purser's desk.

Be aware that many cruise lines are now charging a fee (around 3%) to get money from the casino on your onboard card .

The ATM machines will also charge a fee on top of whatever your bank charges.
