  2. It has been so long ago I can't remember.  LOL.  I would have to look at our Marriage Certificate.  I will bet you will get the answer though.

  3. You have to know what your married name will be as you have to write it on the form, but you will still sign with your maiden name. Not sure if this differs from state to state, so call your local registrars office or talk to your minister.

  4. Maiden...if you are the United States you would still legally be using your maiden name until you went and has your social security card switched.

  5. Your maiden name, its the last time you will legally use it.

  6. You don't sign the Marriage Certificate, you sign the Marriage License, and you would sign that with your Maiden Name.

  9. Maiden. Getting married does not automatically change your name; you must take the steps to legally change it after the marriage. When you get the license you also need to show identification which will obviously be your maiden name, since you havent married yet.
