You can't.  You have to go to one of the centres with sufficient identification and get your photo taken again.  Check to make sure you have the approved ID because without it they will just refuse to process your application.

As the others said, you cannot renew it online.  The reason you need to do this is that you need to provide certain pieces of ID to show you are still eligible for Ontario Health.  Proof of residence (from the approved list-a letter from Grandma Smith wont do) and a supporting document with your name AND signature off the list.  OHIP can ask for more documents if they feel it necessary.

You will also need to have your picture taken again.  The only exception to this is for those people that are in isolated communties in the north and then there are other procedures for them.

Photo health cards have expiry dates on them to help prevent fraud, not to just be a pain.  (new health cards dont have your address on the back and have new security features in them too)

You can't, as every time OHIP the Ontario Health Card expires, it needs to be re-newed with a new picture of you, this helps the government to prevent fraud. So I believe that you will have to go and get it done :)
