  1. It would be a good idea to put any medication allergies on the tags, in case of emergency.

  2. I have been in the Army for over 17 years. Military dog tags have 5 lines of text. 1st line is LAST NAME. 2nd Line is FIRST NAME and MIDDLE INITIAL. 3rd line is SOCIAL SECURITY NUMBER. 4th line is BLOOD TYPE. 5th line is RELIGION.

If you are ordering a set as a novelty item, I would not put your Social Security number on them. Instead of putting your age on them which will change, use your date of birth.

  3. Birth place

  4. In Military dogtags,  you must have your LNAME, FNAME and MI, SSN and GAS MASK size (always everytime people ask Dogtags questions, servicemember and wannabees of the Army always leave out the the gas mask size of either S, M and L. - seems to be that it is always the Army-no offense, but they better be on top of their Dog Tags or else they put in jeopardy of their own troops in NBC warfare)

The Gas Mask size shares the same line as your Blood Type.  If you are a military personnel, you must have that inputted, it is a requirement by all DOD and Armed Forces personnel to have that on, if you don't and nobody cared...then they obviously go throughout their military career not knowing the proper procedures of protocol!

Your DOB (Date of Birth) should NEVER be on your Dog Tags, there is a reason for Operation Security for that reason, but your SSN can apply, you don't add both!  Unless you love Identity Theft!

Plus you input your Blood Type and religion (so you get your rights of your "last rites")

NOTE: Your allergies go on a separate dog tag, they are red...you never put them on your  regular dog tags but ONLY on RED ones.  They are separate and make sure that you have one for each boot and one around your neck.  Any allergies are to be on the Medical Dog Tags and they are RED.
