  1. A memorandum or "memo" as most people call it, is an informal notification or passing-along of information to a specific employee, department, or group...

TO:   put the person's or persons name(s) here

FROM: Your full name and title or department

    DATE: today's date

    RE:  subject of the memo

XC:  xerox copies are going to be sent to maybe the person's boss, department head, or someone else that you just want to notify them that the memo has been sent...

    Body of the memo...

You do not start the body of the memo with "Dear...."  You  just go right into the content of the memo.

Do not end the memo with "Sincerely," as your name is at the top.

One thing you CAN do if you are having  a secretary type your memo is initial next to your name at the top.
