PC gets stuck in a sound loop, monitor goes black.

Question

When playing games, once in a great while my monitor will go black. It will then alternate flashing DIGITAL/ANALOG (the same it does when you first boot the computer). Sometimes theres no sound but sometimes the sound is stuck in a loop from the game i was playing. The only way to recover is reboot. Theres nothing being posted in the event log to indicate theres a problem

I have tried backing down drivers then updating them, and making sure the GPU is not overheating and I am really unable to recreate the problem as its pretty sporadic. I am toying with the idea of returning the Video Card as im sure thats the problem - just not sure if theres anything I am overlooking or not.
