PDF Form - text field help?

Question

I'm trying to make a form for clients, the fill out part is not just a blank line, but instead it consists of letterboxes.. a box for each letter. How do I make the form fill-able so each box will have a letter in it as the user types. For example, if the user types name "John" each letter will be in the first four boxes.

PDF is limited in this.  Try OmniForm 5.0 - It does letter boxes or combs well.  Check out the CMS-1500 insurance form I created with it. Lots of letterboxes. http://fiachraforms.com/quickstart_CMS1500.html.
