    2. i am unsure of what you mean, it could be A or C.  Dosage form is like tablets, capsules, solutions, etc in the form they come in, so A if medication is given as a tablet, or C if the medication is made as a tablet, it sounds like a tricky question to me, but that's what dosage form is
