The insurance is through my husbands work. My name is on the card. Do I put my name... or his?

If it's your husband's policy, you should put his name as the insured, even if your name is on the card. Those darned things are so confusing, aren't they?

You put your husbands name as the person insured if the policy is through his work and his relationship to the patient (you) is "spouse". It's confusing yes.
