  1. On June 1, 2009 , you will need a passport (Canadian) for land travel to and from Canada too.  I  presume you have a green card to be living in the U.S.  ? You can enter Canada by showing your Canadian birth certificate, but you must have a U.S. green card to get back into the U.S.

  2. No, you only need it if you are flying in or out of Canada.  Don't waste your money.

  3. If your method of entry into either country is by LAND (it appears it is as you say you will be driving over the border both times - in and out), you will be fine with just your proof of citizenship (your birth certificate).  An adult needs a birth certificate and valid government issued photo ID, but a minor does not require the latter.  A passport is not required for land travel until at least June, 2009.  At present, a passport is only required for air travel between the countries.

I'm not sure about needing a green card.  From your information, it sounds like you moved to the States as a kid, probably with your folks.  Being a minor, I doubt that you need a green card to get back into the States, but I could be wrong.

One thing you will definitely require, tho, in the event that you are not travelling with your parents, is documentation from both of them (not one or the other but both) giving you permission to cross the border.  If you are travelling with one parent, you still need written permission from the other.  Make sure, as well, that you provide information as to where they can be reached, in case there are questions at the border.  If you are travelling with your folks, you won't have a problem in that regard.
