Passport forms for newborn?

which forms do i need to download for passport for newborn, what is the difference between the one that is passport for minor under 14 and the other one is passport for minor under 16 and i need a notorized parental consent form, since the father will not be there for baby's birth, i searched many websites and each one has differnt type forms, which ones are the right ones? thanks. **(if the father is not a us citizen but has greencard, does he fill out the notarized parental consent form from the us consulate were he lives).**.

  1. Since you said green card, i am think you want the baby for U.S. citizen ship?

    yes the father must notorize the forms in his country and send along with the other forms to the U.S. embassy in his country to be sent to D.C. immigratons that will rule on the babies citizenship.  search for new born childern forms.

  2. When applying for a passport for a baby (or any child under the age of 14--that is the form you will need by the way), the fathers information is only necessary if he is listed on the birth certificate.

    So, if he will not be present for the birth and will therefore not be there to sign to be listed on the birth certificate; his information is not needed and you can apply for the passport alone.

    But if his name IS on the birth certificate at the time you apply for the passport, then you WILL need his signature on the passport documents.  This would be the notarized parental consent form you spoke of.

    You can get the details at the US State Department's website (http://travel.state.gov/passport/get/fir...  and you can always call the nearest office to you which processes passport applications (its the county clerk's office where I live).
