when my husband was a kid he got his last name changed from his dad's to his mom's. i guess it was initated by a letter from social security? subsequently, he got a social security card and driver's license in his mom's last name, but his birth certificate still said dad's last name. anyway, he or his mom has no paperwork to confirm the name change, which we need for him to get a passport. the change was made in california; we now live in new york. is there anyway to fix this without going to california? what office should we call? has anyone had a similar problem?

  1. When I had to get a new birth certificate issued I had to actually go to the state that issued it - even though it was a long way away.

    I would contact the state of California Vital Records division - and find out
