    When water boils it evaporates. This is because you are imparting so much energy to the system that the individual molecules are able to break free and overcome the atmospheric pressure and the density of air.

    A Phase diagram is a mathmatical model which charts the three critical phases of a substance: solid, (sublimation) liquid and gas (evaporation). the point at which these phases are equal is called the triple point. There is also a point is which the gas goes above evaporation and is called the supercritical point. this can be regarded as its own phase, however it is beyond the scoe of your question.
