The same way as for anyone else.  Unless you are overly PC, the term mentally retarded is what it is, no big deal.  I'll tell you, as a mother of a severly disabled child, what I find offensive is all of the PC terms that people use to tiptoe around not "sounding offensive"...

I'm not an expert at this but I believe they need a regular passport like any other child, issued through the government passport office (processed in US at the post office now). You need to fill out an application, get a photo, and apply just as you would for anyone else, no special concessions.

(note -  the term "mentally retarded" is not an acceptable term these days - it is considered derogatory and offensive. Try using special needs, cognitively disabled or developmentally disabled)
