The first sentence says :

"On demand of the concerned party this certificate is issued on unstamped paper for all uses allowed by the law".

"Decorazione" (Decoration) is not fitting the context. Must be mispelled and I guess it's "Dichiarazione" (Declaration).

In the following sentence it must be "visto il" and not "visito al".

The sentence reads " Examined  the nr.1145 of matricula register we hereby certify that ..."

the other sentence (also a bit mispelled) reads :

"Declaration..he/she has attended the course hereafter listed"
