Please explain this joke?

It's meant to be a satire of the high society.  This joke arose when the aristocracy was high class, and another version of this joke's punchline is "the sophisticates."  It's just meant to criticize high society, and that anything is okay when the aristocracy does it (at least that's my take on the joke).  You should watch "The Aristocrats." which is a documentary all about this joke.  That scene appears in it.
