You have something of a puzzle here, but the main clues are the city (Nashville) and "2001 Ohb."  

OHB is local shorthand for Old Hickory Boulevard, also known as Route 254, and 2001 Old Hickory Boulevard is just east of Hillsboro Pike.  

So, TEN probably just refers to Tennessee.  Nashville is pretty obvious, and 2001 OHB is the street address. (Technically, the address puts it in Brentwood, TN, but it is still basically Nashville.)  

Hazmat does not seem to be part of the address.  It means "hazardous materials."  Did you see this on a truck or crate, perhaps?  If so, then the word Hazmat may have just referred to what was inside.
