Please include all schedules? What is this referring to?

My financial aid adviser want me to include ALL SCHEDULES with Form 1040.

As I look through my mom's tax files she has form 4562 that includes 'Depreciation and Amortization'. Does this need to be included?

The basic 1040 Form often has a number of supporting forms and schedules, such as Schedule A for itemized deductions, Schedule C for self-employment income, and so forth.  These are all part of the return, which may run to quite a few pages in some cases.  So all of the forms (copies, actually) should be sent as a complete return.  The 4562 may be part of another schedule, such as a Sched. C or E.
