I agree with the others every child is special, with or without a disability.

I am not sure exactly why you want to write a note, but  if your child has a disability either physical or cognitive, and they have been formally diagnosed, then state the facts as they are e.g;

Dear ?????

Please note my child "name" has been diagnosed with "condition" which affects his/her ability to do ?????, I have enclosed the doctors report, about my child, and what special considerations or adaptions needed to accommodate my child. If you need to discuss this with me please call me on ????????.

If this is a school, then you need to speak to the schools head teacher, and discuss your child's wants & needs are, and what you want, and ask how the school can best support your child.

Please let it be known that my child NAME will be attending school and needs additional attention. He/she IS, OR HAS, WHATEVER THE NEED IS  and may  need one on one attention, at times or( all the time)  Please call me at anytime with questions or concerns. I  would appreciate it very much if we can keep communication open on a weekly basis or daily if need be. Thank you for your time. I am looking forward to working  with you to give my child the best education available.

Write to the school asking for a formal educational evaluation. After they receive the letter, they will evaluate and come to meeting to determine if your child needs any type of special education services. A translator can be provided for you as well as all documents translated into your language.

If you give a few more details we could be extremely more helpful...My son has anxiety and OCD, and I communicate with the guidance councilor weekly sometimes daily, if I can be any assistance, email...

You need an Individual Education Plan (IEP), something where you tell the teachers what your kid's diagnosis is, what his abilities are, what his deficiencies are, and what kind of help you expect from the school system.
