Pokemon D/P; What couse's your Chain to break?

My chains break always at 10-15, it has a reason but i don't know which one. So could someone please make a list of what couses your chain to break?

I'm not that experienced with chaining but I found this great site that clearly tells you how to get longer chains. I think I'll start chaining myself. :-)

Your chain breaks if you dont defeat or catch the pokemon you are up against.
