  1. the only way is to contact the Embassy of Ukraine in the US

    and ask them how to help you out ?

    Ukrainian Embassy in Washington DC, United States

    i found this link too :

    Police Certificate needed?

    Yes.

    How to get one?

    Local Requests

    Police certificates from the Ukraine are available from the regional militia division (Rayonne Viddilennya) of the region of the applicant’s residence permit (Propyska). should present a letter from the Canadian Immigration Visa Office which states their full name (last name, first name and patronymic name), the date of birth of each member of the family who requires a police certificate, and the reason for the request.

    Non-Resident Requests

    You should provide a notarized authorization (Doruchennya) to a representative to apply to the regional militia office on the applicant’s behalf with the request letter from the Canadian Immigration Visa Office. Foreign nationals no longer residing in the Ukraine should inquire about procedures and applicable fees at the nearest Ukrainian embassy or consular office.

    so? the embassy is your first link ;

  2. Unfortunately, the Ukrainian embassy will tell you that you must apply at the regional police station in the region you lived in *in Ukraine*. There is a series of forms you must fill out, and you must provide a considerable amount of documentation proving who you are. This clearance (for the US) must list every name you used previously.

    Depending on the regional "chief of police" (director), you might be allowed to have someone else apply for this document for you, but they will need to provide a copy of your passport and other documents. As the previous poster suggested, you will need a notarized authorization, but it will have to be notarized in Ukraine, or by the Ukrainian embassy in the US.

    It can take up to 4 months to obtain this document, and there are fees, depending on if you want a city, regional, or "all Ukraine" certificate.

    Call the embassy to obtain the number of the regional police headquarters where you lived. I would recommend doing a "Ukraine wide" search, through Kyiv.
