Telling someone that they are the cumshot that their mom should have swallowed instead. I know, it's gross. But it's a very mean insult.

My pick would be:

"Your mother is an hamster and your father smells of elderberries"

But seriously - I don't care if people think I'm ugly or even arrogant (I am), but to be called stupid in any way at all is horrific.

This is one I created for Answers some months back, and it might be in the running.

Use this one for an opinionated loudmouth:

"You must be in constant pain, shooting your mouth off while your head's up your *ss."

A little elaborate, but quite effective.
