Portuguese visa dilemma?

Question

I need to apply for Portuguese student visa. I visited couple of websites and they always give me Schengen visa application form. Is this what I need to fill or is there any other visa application form?

Also, if I am going to be submitting my visa application in Delhi, India (I am Indian), how can I book for an appointment. I called their number million times but they wouldn't pick up.
