Pre Amp Set up

Question

How do I set up Phase Linear Pre Amp to a system that has an amp and a mixer? Where do I go out to in from Pre Amp to Pre Amp/Mixer? Where do I hook up the accessories ie CD player/turntable. All 3 units have ports for these accessories.
