Problems with front bottom teeth.

Okay so awhile ago I went to the dentist about what I thought was a crack shaped like a U on my front bottom tooth. She said it wasn't a crack and there was other white spots (that I knew about before) and it was like bad and I had to use now this certain type of toothpaste. Not to recently I've been trying really hard to get my teeth in better shape but the more I try the worse it seems they get. About 4 or 5 of my bottom front ones are fairly sensitive. I notice sometimes when I like kinda push my tongue against them it kinda hurts like the gum area slightly, or if I bite down. It doesn't hurt if I bite into food but it worries me that like maybe they'll fall out. I'd be embarrassed like no other if that happened and would never smile again. I can't go to the dentist for a few months because my insurance only covers up to 1000 a year and that ran out a couple visits back.. oops. I don't know what to do and I'm really worrying. There's other problems like my whole mouth but I won't get into that. HELP.
