Procedure for name change in Bangalore?

Hi, m residin in b'lore. I want to change my name i.e. add surname to my name. Cud anyone please tell me wat's d procedure for name change n where i can get the forms? and how much time does it take for the whole procedure?

Visit an Advocate..Name will be changed by the court by the process of Afidavit....It takes Nearly 1-2 Months.....
