Product Searches done on Internet,Are results Local,National or Worldwide Results?

I have a website ( Military Flag Display Case. Com) and I believe that with my vast amount of keywords not all searcher's are seeing my listing, in their results. I know that certain keywords land your listing at different locations. The big question is, When I am #1 listing, is that result coming from a national search or is that for a specific quadrant of the United States?
