  1. You may apply for a duplicate school leaving certificate. That is a cumbersome procedure. You may have to file an FIR stating that the certificate has been misplaced. They will give you a certificate that it could not be retrieved. You have to get a non-traceability certificate based on the certificate from the police, from Tahsildar of your locality. With this certificate, you may approach the Educational authorities, who issued the origin school leaving certificate and apply for the duplicate certificate.

  2. PAN card.

  3. To understand what all the Regional Passport authority recognizes as proof of age is to check their official website or information booklet provided along with the passport application form. Proof of date of birth (attach one of the following) Birth Certificate issued by a Municipal Authorities or district office of the Registrar of Birth & death; Date of birth from the school last attended by the applicant or any other recognized educational institute; Or an Affidavit sworn before a Magistrate/Notary stating date of birth by illiterate or semi-illiterate applicants. In your case since the school certificate is lost mentioning her date of birth, either you get a duplicate school certificate issued from the State educational board from where she did her schooling on the basis of the year of her passing or you can use her any other educational certificate where she did her further studies (Calcutta University) if such certificate mentions her date of birth. In your case the Affidavit sworn before the Magistrate/Notary won't be allowed as she is a University Degree holder not illiterate or semi-illiterate lady.

  4. Try to locate from MC records, from schools, colleges, training institutions, from universities and if she is in service from the department where she is functioning and if there is no such entries, then an affidavit from the parents of the woman.
