  1. Many of the better pizzas have more than one cheese.  often 3 or 4.  Provolone is sometimes used.

  2. I live in Ohio and have never had a pizza with provolone, or at least I haven't noticed it having provolone.  It most likely depends on the place you got the pizza.  Some local pizza places can get very unique with their pizzas.  For example, one local pizza chain has a peanut butter and jelly pizza, which is delicious.  It's just the crust, peanut butter, with a layer on grape jelly on top.  One pizza place has a chicken alfredo pizza where the sauce is alfredo sauce.

  3. In St. Louis MO practically all pizza is made with provel, which is a type of provolone.  There is an Italian neighborhood with lots of restaurants and that just became the tradition.  Different states and different countries all have their own varieties of pizza.
