Sounds like schyzophrenia to me.  If this truly seems serious to you, then you're really going to have to help this person out.  While medication can calm the person down, only the love of that person's peers can truly bring them back to reality.  I've never in my life heard of a schyzophrenic getting better because of medication.

I would say, that this person is suffering from one of the following: Schizophrenia, Dis-associative Personality Disorder, or just plain old hallucinations or delusions. (Which can be from drugs, or fever, or other possibilities)

If you know this person, I would suggest they go to a doctor...medical, or a therapist.  

If it is someone you have just noticed on the street, or in a bar, then I would just be cautious around them...you never know what an unstable person is capable of.
