Punchline to the joke in Catch-22?

Catch-22 contains an anecdote that seems like an unfinished joke, and I was wondering if anyone ever figured out what the ending is supposed to be.

"A couple came into my office saying they were having trouble conceiving. They said they tried every night. I checked the girl and she was still a virgin. I then showed them how it was done with dolls, and they thanked me and left. Two days later, the husband returned and punched me in the nose."

And that's it.

I think the couple at the start aren't the husband and the wife, it's the wife and her lover, and clearly the husband wasn't happy that the guy has taught his wife to have s*x with someone else!

It sounds like a funny book.

It appears the couple wasn't married. The woman was married to someone else, this was her lover, and the husband found out what the counserlor did and got mad
