Not necessarily.  Generally means it is a darker roast.

Extra bold is a designation for the amount of flavor in the specific roast of bean.  It will be a stronger more pronounced flavor that will stay on the pallette longer.  It does not have more caffeine because you can have decaf coffee that is extra bold.

well if its extra bold it usually refers to its flavor and/or the caffiene content. the more bold, the more flavor and/or the more caffiene. it all depends on what coffee you choose

no, it actually means it's just roasted more..the less you roast, the more caffeine there is..so if you get something like "light roast" there will be more caffeine in it than "espresso roast"

Well it actually means there is less caffeine in it. The longer coffee is roasted means the flavor it stronger but the actual caffeine is burnt off.
