Question about having a cartilage piercing stretched?

I got my ear cartilage pierced about 4 years ago, it was originally a 16 gauge piercing. However I ended up leaving jewelry out of it for a long time so now I can only put a stud in it. Pretty soon I'm going to be getting another cartilage piercing below it and I was wondering if it's possible to have the first piercing stretched to back to a 18 gauge at the same time? Or should I have them done separately? I've never stretched a piercing before so I'm just now looking into how it works.

Cartilage should be dermal punched not stretched.
