If you see it say 'you pay 0% based on calender year' Does this mean that you pay 0% on whatever the services are- no matter how inexpensive or expensive things can get after paying the out of pocket expense for the calender year?

  1. Hard to tell what you're asking.

Generally, if it says "Based on Calender Year" a policy's description will be referencing an annual deductible.  So you pay $0 AFTER the deductible has been satisfied.  And yes, that means regardless of the cost of the service, so long as you are IN NETWORK.

Call your Agent or Insurance Carrier for more specific details.

  2. please ask more specifically.
