Question about the VOLUNTEER Criminal Records Search (Canada)?

i am volunteering and my volunteer agency gave me a form for the VOLUNTEER Criminal Records Search (as it is needed 4 me 2 b able 2 volunteer...)...

on the form given 2 me, it is asking for my "Five Year Address History"...

i've been here in Canada for only a year and a half so...

do i STILL need to write-in the address in the country where i lived BEFORE??? or do i just need 2 write-in ONLY the addresses where i've lived ONLY HERE in Canada???

had anyone done this b4???

I have had to do a few for different jobs I have applied for. Usually they want your current address & any other address until you hit the 5 yr mark. You may want to double check with the person who gave you the form from the volunteer place though.
