Question about the jonas brothers video question for teen choice awards 2008!!!?

i have the release for it, but i just realized. how do they know if i have it signed of not? on myspace they give me a link so i can print it , but how do i actually submit the release? do i mail it? if so, to what address?

but they've already picked whose questions will be asked.
