Question about the learners permit form in texas?

the white form that you fill out your information on the front and you and your parent sign in the back?

my drivers ed teacher told us to sign some of the back signatures today but it said in a box above to wait until you are in front of a notary public person.

did i basically just mess up the form if i signed my name in two of the boxes? my mom didn't sign any of them.

  2. When my son took drivers ed I did not have to have anything notarized but make sure that you have the following.

    Application for Texas Driver License, DL-14A

    Parental Driver Education Affidavit, DL-90A

    Classroom Instruction Record, DL-91A (You may be required to present this form as proof you have completed all objectives outlined in the Parent Taught Education Program)



    Pay the required fee for Instruction Permit Issuance

    Proof of Liability Insurance (If you own the vehicle)

    Proof of identity

    Proof of Social Security Number

    Verification of Enrollment and Attendance Form in school
