Question on filling out DV Lottery form.

I am having a slight trouble filling out the DV lottery form for my mother.

-Name

-Country of eligibility

My mother is Taiwanese, Born in Taiwan, but is currently a Japanese Citizen under a Japanese Name. Does this mean she has to put in her current Japanese Name, or her Taiwanese name?

Also, in the country of eligibility, the form instructions stated that "normally" the country of eligibility would be the same as the country she war born in. My mother was born in Taiwan, but is currently dwelling in Japan. Which do I put?

  1. Use the name that's in her passport right now.

    You enter her as being from Taiwan because she was born there.

    The DV Lottery is not open for entries right now.  How could you be filling out the form?  It won't be open until October 1st.

    I hope you're not paying some service for this.  Do you understand it's FREE to enter and that there is ONLY 1 official web site that's run by the US State Department?

    The Congressionally mandated Diversity Immigrant Visa Program makes available 50,000 permanent resident visas annually, drawn from random selection among all entries to persons who meet strict eligibility requirements from countries with low rates of immigration to the United States.
