Questionare in pokemon ruby?

Well its supposed to be in the mart, but its not next to the clerk..im confused :3

Anyone know why I can't find it?

Oh and I've filled it out before but I restarted the game.

the mystery gift 1 ? its in the pokemart by the clerk.

it doesnt do anything unless you go to a special event outside the game. if you do it gives you deoxys.
