Questions about USCIS I-751 Form.?

I Have a few questions about the I-751 form..

1. Do I have to do it 90 days BEFORE it expires or do i have to do it IN the 90 days before it expires?

2. In Part 4 it says that i have to fill out the member information of who i gained this status (my wife in this case) under family name do i fill in the last name she carries now (mine) or her maiden name?

The I-751 must be filed NO EARLIER than 90 days before the expiration of the card. This is to remove the conditions of your "conditional" status. You will then recieve a reciept for the application which should extend the validity of your card for one year.  Your wifes maiden name should be indicated. Hope this helps.
