Re-applying for a lost CDIB(Certification Degree of Indian Blood) card?

I have a copy of my original CDIB card. I know for a fact that I am part Cherokee, and I need to get another card because mine got lost. And I want to apply for my daughter one. The only problem is I cannot figure out how to do it online. The website is:

Any help in applying online?? I cannot travel 18 hours at the moment to get it done in person. Also, I need to find a doctor who accepts CDIB cards. They say there is one in Cherokee, NC where I live but I cannot find one?

You might try contacting your tribe of enrollment (assuming you are enrolled).  They'd be able to assist you assuming they have a copy of your original.  Otherwise you'll probably have to go through your local Bureau of Indian Affairs field office to learn how to obtain a copy of your card and apply for one for your daughter.  See http://www.doi.gov/bia/leaders.pdf to find your local BIA office.  Regarding using your CDIB for medical services, you'd have to go to an Indian Health Services clinic.  No doctors I know of have ever accepted my card as it is not an insurance card.  When you think IHS, think HMO.  You will have to go to an IHS clinic and get a referral to an outside provider, assuming that outside provider has agreed to accept IHS as a method of payment.    Good luck!
