Re issue after expiry of minor passport & applicant is still minor(15); what documents are required?

Question

The applicant is 15 years old & has to renew passport. Would turning in the old passport along with birth certificate and parent's passport copies do or should the parents submit any other annexures? The father is no more & the minor is living with his mother now. Hence,Annexure H isn't sufficient. What other documents should be submitted if any?
