Reason for Avianca 52 crashing?

I know it was lack of fuel, but did they find out whos fault it was after investigating? or the closest asnwer?

The single biggest problem was communications. It wasn't made clear to the air control officials how low on fuel the flight was. The pilot simply failed to call for an emergency quickly enough. Other factors pale in comparison to this.
