This is a long term treatment. If staff had to visit the clinic 3 times weekly , think is not fair for the small business .

It depends.  Does your insurance carrier provide for this type of claim or is the business paying it as a sort of self insured situation?  If it is the latter it will depend on why type of agreement has been made with the employees.  Is this a Workman's compensation claim?  Then it would go back to the insurance carrier's policy.
