Coursework: Need Help With Abortion?

I'm doing my Humanities coursework on abortion. What I need to know is if there is a certain criteria or form that needs to be filled in for women to get an abortion. It's very important as it links to my hypothesis and I have looked but can't really find anything useful. Thanks in advance, Georgia.

  1. When you schedule an abortion (at least at planed parenthood) they make two appointments. The first appointment they give you paperwork to fill out, and they do an ultrasound to see how old the embryo is. The paperwork asks questions like how many sexual partners you have had in the last year, the last five years...... they ask if you have had an abortion before, if you ever been pregnant before, if you ever had a vaginal birth. If you have any STD, what is your view on abortion, what is your reason for an abortion (optional). Then after the paperwork they do the ultrasound, followed by some blood work to check if you are anemic then you talk to a doctor about the procedure and decide if you want an anesthetic during the procedure. Then you talk to a counsoler they ask you how you feel. Then the seconed appointment you have the procedure done.
